Trenton, NJ – Medina Health Center has received a $25,000 grant from Horizon Blue Cross Blue Shield of New Jersey. The donation, made through The Horizon Foundation for New Jersey, will fund Thriving at Every Size: A Community-Centered Approach to Obesity and Diabetes Prevention in Trenton.
“This grant from Horizon will enable Medina Health Center to bring inclusive, community-driven diabetes prevention services to our Trenton neighbors,” said Mira Sadeghi, Executive Director of Medina Health Center. “Thriving at Every Size will promote whole-person wellness through free screenings, education, and partnerships that meet patients where they are and respect every individual’s journey.”
The new initiative will offer free health screenings, interactive workshops, and personalized counseling led by Medina’s trained nurse practitioner. The program will build partnerships with organizations such as the Diabetes Foundation to deliver a coordinated and holistic response to diabetes and obesity in Trenton—where deeply entrenched disparities persist.
