At Medina Health Center, we are committed to providing respectful, high-quality care. We value your feedback and take all concerns seriously.

Share Your Concern

Patients and their representatives may submit concerns or grievances regarding their care, services, patient rights, or interactions with our staff.

How to Submit

To submit a formal grievance, please email us at:

grievances@medinahealthcare.org

Our Commitment

All grievances will be reviewed confidentially and without retaliation.

We will acknowledge formal grievances within 10 business days and provide a written resolution within 30 calendar days whenever possible.

Medina Health Center strictly prohibits retaliation against any individual for raising a grievance in good faith.
