Patients and their representatives may submit concerns or grievances regarding their care, services, patient rights, or interactions with our staff.

To submit a formal grievance, please email us at:
Our Commitment
All grievances will be reviewed confidentially and without retaliation.
We will acknowledge formal grievances within 10 business days and provide a written resolution within 30 calendar days whenever possible.
Medina Health Center strictly prohibits retaliation against any individual for raising a grievance in good faith.
